Agreement with processing personal data from questionnaires

1. You hereby give consent to Lentiamo s.r.o. (further on referred to as “the Controller“) with headquarters in Vídeňská 12, 378 33 Nová Bystřice, Czech Republic, IČ 261 04 784, registered at the Regional Court of České Budějovice, section C, part, 14226, to process your personal data which you submit in the questionnaire.
2. In case you answer in the questionnaire contained a special category of personal data, you hereby give your explicit consent in accordance with article 9. par. 2 a) of the Regulation.
3. Personal data given in answers in the questionnaire can be processed, based on your agreement and in order to give information to the Controller which will be used to improve the services provided to customers. Such data will be kept for 5 years.
4. You give your explicit consent with the hereinabove mentioned processing. Providing such personal data is voluntary.
5. This agreement can be terminated by sending a letter by mail or by contacting the company by e-mail.
6. Personal data are processed by the Controller, however, personal data can also be processed in the future by other providers of software, services and applications, which are currently not used by the company.
7. Please be aware that according to the Regulation, you have the right to:
   • Require what kind of your personal data we process,
   • Require explanation about personal data processing,
   • Ask for access to the data and to make us update or correct the data,
   • Ask for deletion of the data,
   • Contact us or the Information Commissioner's Office in case of doubts about complying with personal data protection laws.
